Island Autism Group, Inc. (“Island Autism,” “we,” “us”) is committed to protecting the privacy and confidentiality of the personal information collected through our program applications and registration forms. - Information Collected: We collect personal, financial, medical, behavioral, and contact information for both the applicant and their parent/guardian(s) as detailed our forms. This includes, but is not limited to, names, addresses, phone numbers, email addresses, age, household income, dependency information, medical diagnoses, allergies, medications, physician details, health insurance information, emergency contacts, skill levels, behavioral information, and reference contact details.
- Purpose of Collection: The information collected is used solely for the following purposes:
- To process applications for an Island Autism programs, including our after-school, summer camps and our Community-Based Day Supports (CBDS) program for adults.
- To determine program eligibility, suitability, and appropriate group placement for the applicant.
- To assess eligibility for financial assistance, scholarships, or grants.
- To ensure the health, safety, and well-being of all participants during program activities, including managing medical needs and emergencies.
- To plan and execute program activities effectively, tailored to participants’ needs.
- To communicate with parents/guardians regarding the application status, program details, invoices, emergencies, and other relevant matters.
- To contact the provided educator/clinician reference.
- To coordinate transportation where applicable.
- For internal record-keeping and program improvement purposes.
- Confidentiality and Sharing: We treat all collected information with strict confidentiality. Access to this sensitive information is limited to Island Autism staff and designated volunteers who require it to perform their duties related to program administration, participant care, and safety.
- We may share necessary medical or emergency contact information with emergency medical personnel if a medical emergency occurs.
- We will use the reference contact information provided to contact the named individual.
- We will not sell, rent, or share your personal information with unrelated third parties for marketing or other purposes not directly related to the operation of the Island Autism program.
- Data Security: We implement reasonable administrative, technical, and physical safeguards to protect the confidentiality and security of the information submitted against unauthorized access, disclosure, alteration, or destruction.
- Data Retention: Application information will be retained as necessary for program administration and as required by applicable laws and regulations. Financial information submitted solely for assistance assessment may be retained according to our internal financial aid policies.
